Since 2015 Children’s Medical Group has been recognized as a National Center for Quality Assurance Patient Centered Medical Home. What does that mean for your child? We are the first place to call for care for your child. Dial 203-288-4288 anytime of day. We have office hours 7 days a week with visits by appointment to minimize wait time. When the office is closed, you can reach a provider for urgent concerns or leave a message for the office staff to return the next day when the office opens. Your healthcare providers are familiar with your child’s medical history and can guide you on the best course of action. Call us before heading to an urgent care or the emergency room. Occasionally patients are referred to the Yale New Haven Hospital Emergency Department but many times with guidance your child can wait to be seen in our office the next morning. This can save you time spent in the Emergency Room waiting to be seen with a sick child and will be more cost effective. The office is open Saturday and Sunday mornings to accommodate children who are not feeling well. Leave a message or call early in the morning for an appointment the same day. As concerns arise, children are referred to specialists when needed.

Well Child preventive care is a large part of pediatric health care. Time at well child visits includes age-appropriate screenings for vision, hearing, development and behavior, vaccine administration, sports clearance, health and safety guidance. These visits allow our healthcare providers to establish a relationship you and your family, while also preparing them to care for your child when he/she is not feeling well. If your child is school age, please make sure the preventive well child visit is scheduled at Children’s Medical Group rather than a school-based health clinic.

Our healthcare providers are members of by the American Academy of Pediatrics (AAP) and follow the guidelines set by the AAP and the Centers for Disease Control (CDC) and provide recommended immunizations based on the evidence-based immunization schedule. We only provide care for patients and their families who consent to immunizing their children and will recommend new vaccines as they become available.

 

After your office visit….

You will receive a text or email with a link to review the notes from your child’s office visit and educational handouts or links. To access, you must log into the secure, HIPAA compliant, patient portal. Another feature of the patient portal is sending non-urgent messaging and questions for your provider.

Patient Surveys arrive by text or email following office visits. Please provide us feedback on how we are doing. Did we meet your expectations? Let us know what you like and where we can improve!

Cannot keep a scheduled appointment? Unforeseen events happen and sometimes an appointment scheduled in advance may need to be rescheduled. Please alert us as far as possible in advance. Providers dedicate specific appointment time for your child’s care and if you are not able to attend a scheduled appointment, please allow us to make that time available for another child.
