Transition of Care

Part of the natural course of pediatric care is that children grow into adults and move on to an adult primary care doctor. It is important to request medical records including the immunization record for the new doctor. When requested, your pediatric primary care provider will review your medical records and choose pertinent & recent medical history including test results and specialist notes that your new healthcare provider will want to review along with the immunization record. Medical records are retained for seven years from your last visit.

The same form is used if you or your child is moving your care from our practice to another pediatric group.
